Preschool Forms

Please submit all required Preschool enrollment forms before your child's first day of school:

• The physician's report form (LIC-701) must be filled out by your child's physician. Please download the form here and take it to your child's physician. Some health care providers may issue this form electronically (without the need for an office visit) based on your child's existing health records. Please check with your physician
• Form LIC-701 also includes required vaccination information. Please see here for more information about the requirements.
• We will send you other forms that are required by the childcare licensing agencies electronically via DocuSign.

When you have the completed LIC-701 form, please upload the form in your Ravenna Student Portal in the "Parents" tab under "Enrollment Forms".
